– In order for the dentist to accept reduced DenteMax contact rates, the amount due after the savings have been applied, must be paid at the time of service.

– Savings apply to routine and preventative care like cleanings, exams, x-rays, and fillings, as well as more complex care like extractions, root canals and crowns.

– The Dental Care Savings program also includes savings on dental specialist care, including care from oral surgeons, pedodontists, periodontists, prosthodontists, and children’s specialists.

– Members choose a dentist from the DenteMax Network of over 260,000 participating dentists nationwide and simply save money while getting the same great care.

Since its beginning in 1985, DenteMax has grown to become one of the nation’s largest leasable dental PPO networks serving over 9 million members with more than 260,000 dentist access points. Insurance companies, third-party administrators, and various self-funded groups lease the network for use in their dental benefit plans. DenteMax dentists agree to accept fees below their usual fees. Members experience these savings each time they visit a DenteMax dentist.

Usage Instructions

Step 1 – Get Your Member ID Card

If you don’t already have your FREE Member ID, simply complete the enrollment form and you will immediately receive your Member ID Card via email.

Step 2 – Finding A Participating Dentist Office Near You

In order to locate a participating dentist’s office near you simply use the Find A Provider Tool.

Step 3. Schedule An Appointment

Contact the dentist office directly to schedule an appointment. The telephone number for each dentist office is listed in the Find A Provider result screen. Please tell the office you are eligible for DentaMax Preferred Provider Network discounted care.

Step 4. Visit Your Selected Dentist Office For Dental Care

Attend your scheduled dentist appointment, and present your Member ID Card at the time of service.
