Simply present your Member ID card at the time of service, and inform the vision center that you are an active member of the Diverse Health Benefit group’s EyeBenefits discount plan, and your vision center will reduce the billing rate to the pre-negotiated EyeBenefits rates.
INSTRUCTIONS ON HOW TO OBTAIN VISION SAVINGS
Step 1
Complete the enrollment form and you will immediately receive your Member ID Card via e-mail.
Step 2
Find a participating vision provider near you by using the Find A Provider Tool (below).
Step 3
Schedule an appointment with the provider’s office directly. The phone numbers can be found on the Find a Provider results screen. To obtain information as to the amount of discount you will receive, just tell the provider’s office that you have an EyeBenefits discount card and ask what discount they provide EyeBenefits members.
Step 4
Visit your selected provider’s office for care. Present your Member ID Card at the time of service. If an optical location is uncertain if EyeBenefits is in their system, please have them call EyeBenefits Provider Relations directly at 480-659-2849. All optical locations listed in the Provider Lookup have been contracted and credentialed with EyeBenefits.
